Discharge Planning

The discharge planning process is implemented at the time of admission. It is a dynamic, on-going process.

The treatment team will address the problems and concerns of the patient and will offer suggestions for the continuation of treatment after discharge. This may include but would not be limited to a partial-day program, AA/NA attendance, intensive outpatient programming, outpatient visits with a private therapist, and/or follow-up with a psychiatrist.

Discharge occurs when goals are achieved, nursing interventions are complete, and the patient withdraws from treatment or the physician requests a discharge.
